Forms | OIT-ELN Requests
OIT-ELN uses online forms to streamline as well as document your instructional technology requests, applications, and inquiries. If there's something you're trying to get, do, or instigate online, we've probably got a form for it:
- Copy Course Content
Use this form when you need help transferring content from one course to another--or from a current iteration of your course back to its master copy. - Establish Course Master Copies
Use this form to establish a master copy for the first time. You may establish a "blank" shell or request to have an existing course copied into the shell. Doing this will assure that your courses do not "disappear" from the server when they are archived annually. - Instructional Technology Challenge Grants
Find out more about instructional technology grant opportunities; download the application guidelines and sample grants. - Request Graphic Design Assistance
Use this form to request assistance developing graphic and media elements for your courses, including course banners and icons. - Request Course Review
Use this form to initiate a Quality Matters- or Monterey Institute-style review of your hybrid or fully online course. - Request a Course Template
Use this form to have your course or courses populated with a course template (designed for hybrid and fully online course developers). - Request a New Course Shell/Master Copy
Use this form to request a course shell for a new course you are planning to offer but wish to build now. - Request Respondus
Use this form to request the popular assessment-development software, Respondus.
